Waiver & Health Declaration Forms
We continually strive to improve our services and member care. As part of this, we are in the process of updating our member records and it is possible we require a current Health Declaration & Waiver form from you which is a requirement of our Club Rules.
It takes less than three minutes to complete so please stop at Reception or the Gym Desk on your next visit to the Club. The information will be treated confidentially and be filed against your individual membership record. The form must be one printed by the Club and completed in person so we are unable to send an email version for your completion.
